Terms of Service — My Glow To Nurse

1. Our Services

My Glow To Nurse provides botox and aesthetic treatment services on an appointment basis (“Services”). These Terms apply to all clients who book, receive, or inquire about our Services.

2. Eligibility

You must be at least 18 years old to book an appointment or use our website. By using our Services, you represent that you meet this requirement and that any information you provide, including health history relevant to your treatment, is accurate and complete. We reserve the right to decline Services to anyone who does not meet eligibility or medical suitability requirements.

3. Appointments, Cancellations, and No-Shows

• Appointments may be scheduled online, by phone, or in person.
• Cancellations must be made at least 48 hours in advance. Late cancellations or no-shows may be subject to a fee of $100, charged to the payment method on file.
• We reserve the right to reschedule, decline, or discontinue treatment at our clinical discretion, including for medical or safety reasons, without liability for the resulting delay.

4. SMS/Text Messaging Program

By providing your phone number and separately opting in verbally, via a dedicated checkbox on our web form, or by texting our opt-in keyword, you consent to receive text messages from My Glow To Nurse related to appointment confirmations, reminders, changes, cancellations, and customer support. This consent is collected and recorded separately from your general agreement to these Terms.

• Message frequency: Approximately 2–4 messages per month, depending on your appointment activity.
• Message and data rates may apply.
• Opt-out: Reply STOP at any time to unsubscribe. You will receive one final confirmation text.
• Help: Reply HELP for assistance, or contact us at [support email/phone].
• Consent to receive text messages is not a condition of purchasing any Service.
• Carriers are not liable for delayed or undelivered messages.
• We do not sell or share your phone number with third parties for their own marketing purposes.

5. Medical Treatment, Informed Consent, and Assumption of Risk

• Our Services are administered by qualified medical professionals, but nothing on our website or in these Terms constitutes medical advice, diagnosis, or a guarantee of outcome.
• Prior to any treatment, you will complete a separate written informed consent specific to that procedure. That clinical consent document — not these Terms — governs disclosure of treatment-specific risks, contraindications, and expected outcomes. If there is any conflict between these Terms and your signed clinical consent, the clinical consent controls as to treatment risk and care.
• Nothing in these Terms limits, waives, or releases the Company or its practitioners from liability arising from professional negligence, malpractice, or failure to meet the applicable standard of care. Any provision of these Terms is void to the extent it purports to do so.
• You are responsible for disclosing complete and accurate health history prior to treatment. Failure to disclose relevant health information may affect treatment safety and outcomes.
• Results vary by individual; we do not guarantee specific cosmetic outcomes.

6. Photos and Media

If we photograph or record you for clinical charting purposes, those images are treated as part of your treatment record and handled per our Privacy Policy. We will only use before/after photos or other images for marketing, social media, or promotional purposes with your separate, explicit, opt-in consent, which you may withdraw at any time.

7. Payments

Fees for missed or late-cancelled appointments are described in Section 3.

8. Intellectual Property

All content on our website (text, images, logos) is owned by or licensed to My Glow To Nurse and may not be copied or used without permission.

9. Limitation of Liability

To the fullest extent permitted by law, and except as provided in Section 5 regarding professional negligence and standard of care, My Glow To Nurse is not liable for indirect, incidental, or consequential damages arising from your use of our Services or website.

10. Dispute Resolution and Governing Law

These Terms are governed by the laws of the State of Washington, without regard to conflict-of-law principles. Any dispute arising from these Terms will first be addressed through good-faith negotiation, and if unresolved, may be brought in the state or federal courts located in King County, Washington.

11. General Provisions

• Severability: If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect.
• Entire Agreement: These Terms, together with any clinical consent forms you sign, constitute the entire agreement between you and the Company regarding your use of our Services.
• Assignment: We may assign these Terms in connection with a merger, acquisition, or sale of assets. You may not assign your rights under these Terms without our consent.
• Changes: We may update these Terms from time to time. Continued use of our Services after changes are posted constitutes acceptance of the revised Terms.
